Information you provide

This website may allow you to enter your name, phone number, email address, state, coverage scope, and preferred callback time. When hosted on Netlify, the quote-request and coverage-path forms are processed through Netlify Forms. Form submissions may be stored in the site owner’s Netlify account and used to respond to your inquiry.

How information is used

Information you send may be used to respond to your inquiry, discuss health coverage options, schedule a call, and provide service-related follow-up.

Sensitive information

Do not submit medical records, Social Security numbers, banking information, policy numbers, account credentials, or other sensitive personal information through the website form or ordinary email.

Phone calls and text messages

By voluntarily contacting the number shown on this website, you consent to receiving a response regarding your inquiry. Message and data rates may apply. Consent to communicate is not a condition of purchasing insurance.

Advertising and lead-source attribution

The website may record campaign source, medium, campaign name, content, referral code, landing page, referring website, and advertising click identifiers when present. This helps measure which marketing sources produce inquiries.

Lead identifiers and duplicate control

Website forms may generate a unique lead identifier, submission timestamp, form variant, and duplicate-control key. These values help organize inquiries and reduce accidental duplicate processing.

Scheduling prefill

When a Cal.com scheduling link is configured, information already entered by the visitor—such as name, email, phone, state, coverage scope, or a general quiz summary—may be added to the booking URL to reduce repeated typing. The scheduling platform’s privacy practices also apply.

First-party conversion events and experiments

The website may record first-party events such as quiz starts and completions, form starts and submissions, scheduling clicks, phone or text clicks, report downloads, renewal requests, and installation of the website app. Event records can include a randomly generated visitor or session identifier, campaign source, page, and assigned A/B-test variant. The site is configured to respect a browser Do Not Track signal when enabled.

A/B testing may display different button wording to measure which presentation helps visitors complete a useful action. The assigned variant can be stored locally in the browser for consistency.

Saved form and appointment progress

Basic contact fields and appointment-preparation details may be saved locally in the visitor’s browser so an unfinished request can be continued later. Sensitive fields are excluded. Local data can be cleared from the displayed recovery control or through browser storage settings.

Installable website app and offline cache

The website can be installed to a device home screen. A service worker may cache public website files for faster loading and limited offline access. Forms and current information still require an internet connection.

Local preparation workspace and readiness score

The doctor, hospital, prescription, pharmacy, question, and readiness tools can save information only in the visitor’s browser when the visitor selects the local-save action. This information is not submitted automatically with a website lead form. Visitors should not store Social Security numbers, banking information, passwords, policy numbers, or complete medical records in these tools.

Credential summaries

The License Verification Center and wallet card display privacy-safe summaries of credential information supplied by Dustin. Original credential images containing a residential address are not published through these features.

Cal.com scheduling

Scheduling buttons open the connected Cal.com event at cal.com/dustinhealthadvisor/retell-health-coverage-review. Cal.com receives the information a visitor submits through its booking form and may receive permitted prefill and campaign parameters. Live slot cards use a server-side Netlify function so the Cal.com API key is never placed in browser code.

Coverage Studio, resume links, and decision journal

The Coverage Studio stores plan-comparison inputs, non-sensitive profile selections, and decision-journal text locally in the browser. Privacy-safe resume links contain only state, household scope, situation, priority, and progress. They exclude provider, prescription, phone, email, and journal details.

V16 local planning tools

Coverage fingerprints, priority sliders, comparison entries, Provider Access Passport records, achievements, advisor product records, and quote drafts can be stored locally in the visitor’s browser. They are not automatically sent through public lead forms. Visitors should erase shared-device data and avoid storing Social Security numbers, banking information, passwords, policy numbers, complete medical records, or other highly sensitive information.

Callback and booking-recovery requests

Callback and booking-recovery forms are submitted only after the visitor provides contact consent. Automatic delivery of a scheduling return link requires a separately configured workflow.

Third-party services

Your hosting provider, email provider, analytics tools, advertising platforms, or other services added later may process limited technical or contact information under their own privacy policies.

Local browser preferences and preparation tools

The website may use local browser storage to remember optional display settings and to recognize that a visitor has returned after a period of time. This information remains on the visitor’s device and is not submitted as a lead.

The doctor, provider, hospital, pharmacy, and prescription preparation tool creates a personal checklist locally in the browser. Entries in that tool are not transmitted to Dustin, Netlify, or the website unless the visitor separately chooses to share the downloaded or printed list.

Cookies and analytics

This website package does not include advertising pixels or analytics by default. If those tools are added, this policy should be updated and any legally required consent mechanism should be enabled.
